Use this New Patient Checklist to ensure we receive everything we need to get you started on your journey to better health.
____ Complete your Electronic Paperwork. It must be completed 1 WEEK prior to your appointment in order for us to review all pertinent information.
This includes ALL of the following:
1) New Patient Information
2) New Patient Medical History Questionnaire
3) Medical Records Release
4) Patient Privacy Rights Form
____ Once we receive your electronic paperwork our office will contact you to verify your New Patient appointment. If we have not received your new patient paperwork at least one week prior to your appointment, your appointment will be considered cancelled by you.
A note about insurance...we do not bill your insurance for our services. However we will issue you a SUPERBILL: that you may submit to your insurance yourself. We will even provide the envelope! Please note that your appointment may not be 100% covered and if you have questions regarding how much they will pay, you will need to contact your insurance provider directly.
